How to add customer survey to site?

if it is small survey then you can go for polldaddy.com free version.

You can add a survey from within OLSB, but currently you can not show the results on your web site.  Goto Business Applications, Your working WorkSpace, click "+Add", then select "Survey" and type your questions, publish the Survey Form to your page.  If your survey is small you could copy and paste your survey results from the workspace survey tab to your web page but you would need to do this on a regular bases as I did here: http://wynnwoodhoa.community.officelive.com/WebSiteSurvey_Form.aspx  .  Thats why a third party survey is always recommended.
